TAConnect is actively supporting the Transforming Health and Resilience for Individuals Through Vibrant Empowerment (THRIVE) Project, a five-year USG-supported initiative that began on 9 December 2024. Implemented by the Center for Clinical Care and Research Nigeria (CCCRN) in partnership with Project Hope, Heartland Alliance, and community based organizations , THRIVE aims to improve the health and wellbeing of children and adolescents living with HIV while reducing HIV-related risks among uninfected young people, contributing to sustained epidemic control.
Collaborating closely in 16 Nigerian States with CCCRN and Heartland Alliance, TAConnect leverages government structures and partners, such as State Primary Health Care Development Agencies, State Committees on Food and Nutrition, State Health Insurance Agencies, civil society organisations, and implementing partners, to deliver integrated , community-centered solutions. TAConnect is also contributing to strengthening households and high-risk subpopulations by expanding access to integrated reproductive, maternal, newborn, child, and adolescent health, nutrition, and malaria services.

TAConnect strengthens community systems for integrated service delivery by building the capacity of CSOs, supporting community health agents, and linking children, adolescents, and other vulnerable populations to quality RMNCH services. Through engagement and linkages with government platforms such as MNCH Week, BHCPF, MAMII Multiple Micronutrient Supplement programmes, and Supplementary Immunization Activities, TAConnect drives community mobilization, demand generation, referrals, and accountability through strengthened linkages to facilitate improved access and utilization of quality services structures, including Ward Development Committees.
Within the first months of project implementation, TAConnect has attained some key achievements. Pregnant adolescents living with HIV have been linked to quality antenatal and delivery services at supported facilities across both MAMII and non MAMII LGAs 31 CSOs across the North and South received in-person and virtual mentoring, RMNCH services have been integrated into the existing case management system, and effective linkages between ART clinics and RMNCH services were established at facility levels.
Through these efforts, TAConnect contributes to ensuring that children and adolescents living with HIV, and their households, have improved access to lifesaving services, contributing to stronger health systems, better wellbeing, and sustainable impact across Nigeria.
